KETOFOL use in CUH ED only with EM consultant oversight
Ketofol 1:3
- 20 mL syringe
- Draw up 5 mL of Ketamine 1% (50 mg)
- Add 15 mL of Propofol 1% (150 mg)
- Mix well by rolling … now you have Ketofol 1:3
- Ketofol 1:3
- Every mL = Ket 2.5 mg; Prop 7.5 mg

Ketofol 1:3 ADULT dosing
- Adult Loading Dose: 0.05 mL/kg
- Top-up doses: ½ LD, Q2min
Example:
Patient 20y, 80kg — # reduction, cardioversion:
- LD: 4 mL at start
- Then Top-up: 2 mL every 2 min

- This is a dosing guide only (fit adults)
- Dose-adjust for each patient and procedure
- If in doubt; start low dose, top-up often
Reduce loading dose if:
- Elderly, unwell, intoxicated, ↑BMI
Dose frequency:
- As per gestalt, patient sedation & comfort
Ketofol 1:3 CHILD dosing
- Child Loading Dose: 0.1 mL/kg
- Top-up doses: ½ LD, Q2min
Example:
Patient 2yo toddler, 12kg — # reduction, I+D:
- LD: 1.2 mL at start
- Top-up: 0.5 mL every 2 min
